ORDER
ARJET SEALTIN DEMIS through counsel, filed the above-captioned verified petition, praying that the erroneous entry as to her date of birth be corrected from “May 28,1985”; and the missing entries as to her middle name and that of her mother be supplied with the entries “Saltin” and “ Licawen” respectively, all in her Certificate of Live Birth bearing Reg. No.2001-870, on file with the record of respondent offices.
Petitioner further alleged that she was born on May 28, 1995 to Spouses Anthony Demis and Alisia Licawen Saltin at Buguias, Benguet. A copy of her Certificate of Live Birth is attached to the Petition as Annex “ A” . In said record of birth, her date of birth was erroneously entered as “ May 28,1985” , instead of “ May 28, 1995”; and her middle name and the middle name of her mother are not indicated. Hence, the instant Petition.
WHEREFORE, finding the petition to be sufficient in form and substance, initial hearing is hereby set on August 26,2014 at 9:00 in the morning, at the Session Hall of this Court, located at the 3rd floor, Buguias Municipal Hall, Abatan, Buguias Benguet, and all persons interested may appear and show cause, if any, why this petition should be granted.
Let this order be published at the expense of the petitioner in the THE JUNCTION. A newspaper of general circulation in Baguio City and Benguet province, once a week, for three (3) consecutive weeks, the first publication to be made not later than July 6, 2014
Furnish a copy hereof, together with a copy of the Petition and its Annexes, to the Office of the Solicitor General, Makati City; the Provincial Prosecutor of Benguet: the Local Civil Registrar of Buguias, Benguet; and the National Statistics Office, Manila, fort their information; as well as, petitioner and her counsel.
