NOTICE OF INITIAL HEARING
A verified petition was filed by Baby Boy Dogue Culki a.k.a Marlon Dogwe Nagangi, praying that after due notice, publication and hearing an order or judgment be issued directing the Local Civil Registry of Baguio City and the National Statistics Office to change his name from Baby Boy Dogue Culki to Marlon Dogwe Nagangi..
In his petition, he alleged among others that he is of legal age, Filipino citizen and presently residing at in Purok 1, Dontogan Barangay, Baguio City; that private respondents Joseph Tomog Culki a.k.a Jose Tomog Nagangi and Mary Ngayawan Dogue a.k.a Mary Ngawayan Dogwe are the petitioner’s parents; he was born on December 18, 1970 and his birth was registered with the Local Civil Registry of Baguio City indicating his name as Baby Boy Dogue Culki, ; he intends to correct his registered name, to Marlon Dogwe Nagangi, for the reason that the same is ridiculous; he also desires to correct the spelling of this middle name from Dogue to Dogwe, considering that his parent’s marriage contract indicates his father’s name as Jose Nagangi and Mary Dogwe; he further alleged that he had been using the name Marlon Dogwe Nagangi since his childhood; he used the said name when he entered elementary at Virac Elementary School, Tuba, Benguet and during high school at Immaculate Conception School, Banaue, Ifugao; he likewise claimed that he used the said name when he registered as a voter before the office of the Election Officer at Hingyon, Ifugao; when he secured his Social security membership, he also used the name Marlon Dogwe Nagangi; that to avoid any inconvenience and confusion, and likewise to conform to all the records which reflect his true identity, he desires to have his name in his Certificate of Live Birth be corrected.
WHEREFORE, finding the petition to be sufficient in form and substance, let the same be set for hearing before this Court located at Room 313, 3rd floor of the Justice Hall, Baguio City on December 5, 2014 at 9:00 o’ clock in the morning at which place, date and time, the petitioner shall be allowed to substantiate his petition, and any interested person may show cause, if there be any, why the petition should not be granted.
Let a copy of this Notice of Initial Hearing, together with a copy of the petition and its annexes, be sent to the Honorable Solicitor General, Makati City, Metro Manila, the City Prosecutor of Baguio City and the respondent Local Civil Registrar of Baguio City. Also, furnish copies of this notice to the National Statistics Office, Manila, the Office of the Clerk of Court for publication, and the petitioner, through his counsel, Atty. Premier Dee Ewigkeit C. Castro, of the Public Attorney’s Office, Baguio City.
Also, let this Notice of Initial Hearing be published at the expense of the petitioner, once a week for three (3) consecutive weeks in a newspaper of general circulation in the City of Baguio and the whole Cordillera Region, the last publication of which that shall not be later than the date of initial hearing.
The Proess Server is hereby directed to post copies of this notice on the bulletin boards of this Court, the Justice Hall, the City Hall, the City Market and the Post Office, all of Baguio City.
