ORDER

MARISSA T. PACIO –LANGBAYAN, also known as Feliza T. Pacio, through counsel, filed the above-captioned verified Petition, praying that her second registration of birth bearing Reg. No. 93-840 on file with the respondent Civil Registry Office of Mankayan, Benguet, be cancelled. Further, to order the change of her first name from “Feliza” to “Marissa”, in her Certificate of Live Birth bearing Reg. No. 86-1150, on file with the same Registry Office.
Petitioner alleged that she was born on October 15, 1986, at Mankayan, Benguet, to Spouses Magno C. Pacio and Julia P. Teweng-Pacio. A copy of her Certificate of Live Birth issued by the respondent Offices are attached to the Petition as Annexes “A” and “B”. From the time she entered formal schooling up to the present; Petitioner had been using and is known by the first name “Marissa”, as shown by copies of her Certificate of Baptism , Secondary Student Permanent Record and Marriage Contract (Annexes “D”, “F” and “E”, respectively). Lately, she discovered that she has another record of birth, bearing her first name as “Marissa”, as shown by a Certificate of Live Birth bearing Reg. No. 93-840, existing in the records of the respondent Local Civil Registry Office; hence, the instant Petition.
WHEREFORE, finding the Petition to be sufficient in form and substance, initial hearing is hereby set on June 23, 2015 at 9:00 in the morning, at the Session Hall of this Court, located at the 3rd Floor, Buguias Municipal Hall, Abatan, Buguias, Benguet, and all persons interested may appear and show cause, if any, why this petition should not be granted.
Let this Order be published at the expense of the petitioner in the THE JUCTION NEWSPAPER , a newspaper of general circulation in Baguio City and Benguet province, once a week, for three (3) consecutive weeks, the first publication to be made not later than January 18, 2015.
Furnish a copy hereof, together with a copy of the Petition and its annexes, to the Office of the Solicitor General, Makati City; the Provincial Prosecutor of Benguet; the Local Civil Registrar of Mankayan, Benguet; and the National Statistics Office, Manila, for their information; as well as, petitioner and her counsel.
